1. Have you traveled internationally within the last 14 days?

3. Have you had a new fever (100.4* or higher), or a sense of having a fever (ie. sweating, chills)?

4. A new cough that you cannot contribute to another health condition?

5. New shortness of breath that you cannot attribute to another health condition?

6. A new sore throat that you cannot contribute to another health condition?

7. New muscle aches that you cannot attribute to another health condition or that may have been caused by a specific activity (such as physical exercise)?

IF YOU CAN ANSWER YES TO ANY OF THESE QUESTIONS, PLEASE DO NOT SCHEDULE AN APPOINTMENT AT THIS TIME.
